I spotted a couple of errors:
#22: the second person plural, subjunctive mood, imperfect tense, of the verb 'essere' (to be) is not "Che voi siate", but "Che voi foste" (check it here: https://www.thoughtco.com/italian-verb-conjugations-essere-4093070). "Che voi siate" is present subjunctive.
#24: the question asks for the first person plural, subjunctive mood, past tense, of the verb 'avere' (to have got): the correct answer is "che noi abbiamo avuto", not "che io abbia avuto" (which is instead the 1st person singular) (check it here: https://www.thoughtco.com/italian-verb-conjugations-avere-4093137)

Also, but this is more subtle: 
#4: the correct Italian translation of "happy birthday" is "buon compleanno", as "tanti auguri" can be used for a wide range of events and occasions (check it here https://www.clozemaster.com/blog/happy-birthday-in-italian/)

In addition to those in the comment above, I'd like to add one more inaccuracy I've spotted.
#21 In the question it is asked to conjugate the verb 'camminare' (to walk), but in the answers there are conjugations of the verb 'andare' (to go).
